HBPC Physician Assistant/Nurse Practitioner

The HBPC is patient centered. The unit of care is the Patient and Caregiver, if present. Their needs and preferences guide the goals of the care and treatment plan. HBPC targets primarily the following three types of patients in need of home care:
Longitudinal care patients with chronic complex medical, social, behavioral conditions, particularly those at high risk of hospital, nursing home, or recurrent emergency care. Longitudinal care patients who require palliative care for an advances disease that is life limiting and refractory to disease-modifying treatment. Patients whose home care needs are expected to be of short duration or for a focused problem, when such services best help the facility meet the needs of this population. Supervisory Controls:
The Provider is directly responsible to the HBPC Medical Director in the assigned area and must hold a collaborative agreement with a supervising Physician. Work Schedule:
Monday - Friday, 0800 to 1600 hours. The HBPC Program provides vehicles and supplies for travel and provision of primary care to Veterans in their homes. Unrestricted Driver's License required. Telework Eligible:

Nurse II/III:

Collaborates with others in research activities to validate and/or improve care. Basic Requirements - Nurse Practitioner U.S. Citizenship, Birth or Naturalized (appropriate documentation required) Master's degree from an NLN or CCNE accredited Nurse Practitioner Program Certified by the National Commission on Certification of Physician Assistants (NCCPA). Current, full, unrestricted license as a Registered Nurse State approval to practice as a Nurse Practitioner Basic Requirements - Physician Assistant Must be a citizen of the United States. Must have a Bachelor's Degree from a Physician Assistant Training Program which is certified by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) OR graduation from a PA training program of at least 12 months duration which is certified by ARC-PA and a Bachelor's Degree in a health care occupation or a health related science OR graduation from a PA training program of at least 12 months duration which is certified by ARC-PA and a period of progressively responsible health care experience such as an independent duty Medical Corpsmen, Licensed Practical Nurse, Registered Nurse, Medical Technologist or Medical Technician. The duration of approved academic training and health care experience must total at least 5 years. Must be certified by the National Commission on Certification of Physician Assistants (NCCPA). Must be proficient in spoken and written English as required by 38 USC 7402(d), 7407(d). Preferred Qualifications Minimum 2 years experience in Home Based Primary Care. Prior experience working with Veteran patients in community or hospital setting. Demonstrated evidence of leadership potential with an emphasis on skills in interpersonal relationships, program development, critical thinking, and problem solving. Documented evidence of successful professional practices with progressively complex positions/assignments. Demonstrated ability to use computer systems in support of patient care delivery. Demonstrated excellence in written/verbal communications, time management and working with an interdisciplinary team. The position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring Veterans and objects may be required. May be required to don protective equipment in isolation situations or operative/invasive procedures. Responsible for the reprocessing of reusable medical equipment, according to the medical center policies. Required to use available safe patient handling lifting devices in all clinical areas. May also be exposed to Veterans who are combative, secondary to delirium, dementia, or psychiatric disorders.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
