Data Collection & Tracking Support The primary purpose of this position is work involving the collection, compilation, and/or tracking of data for statistical information on admissions, discharges, falls, infections, hospitalizations, emergency department visits, and incidents in support of the organization's program operation. The incumbent develops and maintains tracking spreadsheet data with patient information such as admission and discharge information, length of stay, acuity, IDC scheduling, clinic visit scheduling, Do Not Resuscitate (DNR) status, and other essential clinical information; this data must be accurate and updated on a daily basis to be used by HBPC and the Medical Transitional Care staff to provide patient care. The incumbent generates computer forms as needed to facilitate projects, system changes and to improve effectiveness and efficiency within HBPC and home care. Prepares the agenda for the HBPC monthly team meetings, prepares and maintains care plans and follows up on non-clinical issues. Participates in the HBPC Performance Improvement activities, assisting staff with special PI projects, facilitates components of the tracking program, prepares materials for patient satisfaction surveys and assists with coordinating and performing quarterly clinical record reviews. Independently performs a variety of office functions, including but not limited to:
requesting office supplies, distribution of mail, researching, preparing and tracking request for purchase and/or repair of office and clinical equipment. Coordination of information, Office Support The incumbent manages all aspects of staff workload and ensures that the productivity of HBPC is captured in a timely manner. This position requires using VISTA and CPRS, establishing, reconciling and maintaining provider clinics and the diagnosis and resolution of computer and human variance. The incumbent trouble shoots problems and; keeps abreast of changes in VA patient information and resource utilization requirements; generates computer data bases and develops forms to assist management and staff with program evaluation, resources utilization and operational efficiency; and, further contributes to evaluation of program effectiveness by collating data and patient information, identifying trends, local situations, program or staff changes and their effects on the HBPC program. Functions as ADPAC for OIT requests and first point of contact for problem resolution for technology. The incumbent maintains HBPC patient records and facilitates the provision of care by monitoring and informing staff of inpatient and urgent care activity and maintaining the VISTA provider file. The incumbent is the point of contact for all consults/referrals to home care; is responsible for ensuring all phone calls to the HBPC office are received and answered promptly. The incumbent assists with travel arrangements, interfaces with Prosthetics, Pharmacy, lab and diagnostic services. Ensures enough supplies are available for the program staff, and maintains vehicle logs, trip tickets and is the point of contact for GSA leases for program Office Supplies and Equipment Obtains and monitors the use of services, vehicles, supplies, or equipment for the office. Provides general office supply and equipment support services, such as obtaining standard office supplies and services, procuring or justifying the full range of office services, or answering questions concerning policies and procedures related to support/office services. Consolidates equipment or service requests from HBPC staff to determine usage and needs and merges needs into one recommendation for the organization as a whole when requested. Subsequently, acquires the equipment and services using procedures to procure, authorize, control and justify these purchases. Maintaining Appointments, Calendars and Schedules Coordinates meetings, and/or schedules conferences. Provides general calendar maintenance clerical services such as maintaining calendars and recording appointments, arranging for meeting and small conference administrative details, or following up on action items with appropriate staff members independently. Attends meetings, notes commitments made, informs staff of commitments. The incumbent arranges for meeting space, and support arrangements for meetings/conferences. Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to:
Providing administrative, clerical, or secretarial support to a medical office, which requires knowledge of medical terminology and scheduling; Providing administrative, clerical, or secretarial support to a program office; Experience that provided the opportunity to make decisions, judgments, and evaluations to support a program; Experience identifying and addressing administrative issues independently; Experience compiling, consolidating, and using automated data processing equipment to generate a variety of correspondence and reports; and Special skills in word processing and Microsoft office products successfully, ordering supplies.
